Permanent Supportive Housing Program (PSHCM)

The Permanent Supportive Housing Program utilizes the Housing First approach in responding to the needs of vulnerable families and individuals experiencing homelessness. The Housing First approach is the commitment to provide housing to those experiencing homelessness as quickly as possible without creating prerequisites to being housed, such as participation in drug treatment programs. Qualified Intellectual Disabilities Professional (QIDP)

The QIDP is a social worker, or human resources professional with specific higher education accomplishments and experiences that enable him/her to ensure that the services received by a people from the Developmental Training and residential programs in which that person is enrolled are consistent with the person wishes, needs, and desires (as documented in that person’s individual service plans), and funding agency requirements (if any). ACT Clinical Manager

The ACT Clinical Manager directs the operations of the Assertive Community Treatment (ACT) Services Team. The Team Leader supervises a team comprised of a nurse, generalist position, addictions professional and recovery specialists for at least 80 consumers. The team is responsible for providing Assertive Community Treatment Services (ACT) in accordance with the Dartmouth Fidelity Model (1998) the needed treatment, rehabilitation, and support services to ACT consumers with mental illnesses. The ACT Team Leader evaluates the team in conjunction with appropriate psychiatric support to ensure excellence through courteous, helpful, and respectful services to program consumers.

Recovery Support Specialist/CAC
