Job Description


Ensures the well-being of clients with coordination of care within day program/natural home or group home and promotes their development. A key component of this job is to assist clients in their physical, social, emotional and daily like skills development. This increases their independence and allows them to function appropriately in the community.

Provides support to adults in care. Services provided include crisis intervention, behavior management, and life skills training, whether on a one-to-one basis or in a group environment.

Plan, develops, implements and evaluates recreational, social and/or educational activities for clients served by the organization.

The worker strives to create a fun and enjoyable atmosphere to encourage client participation in  programs.


Key Duties and Responsibilities:

Direct clinical and consultation services in accordance with DBH Policies and Procedures policies and procedure of facility, and in accordance with the ethics and standards of relevant professional organization (e.g., DBH, and all Government agencies.

Interrelate and work effectively with facility staff, and outside support agencies as delegated by Clinical Supervisor, or Clinical Director. Weekly supervision meeting with all community support workers.

Maintain the confidentiality of individual information in accordance with DBH policy and procedure, state law, site policy and the standards of District Government if those accrediting bodies are applicable to the site.

Completion of specific duties and responsibilities as designated by the Mental Health Coordinator, Clinical Supervisor or Mental Health Director. Designation of duties will be determined by current needs of the consumer and the mental health professional privilege status, taking into consideration employee’s interests whenever possible.

Provision of crisis intervention services to consumers as referred by medical staff or to-self referred individual.
Crisis intervention may require consultation with staff regarding management /treatment concerns, referral, and /or mental health follow-up. Crisis intervention duties may be assigned by the Mental Health Coordinator, Completion of Clinical Services Repots assigned by the Mental Health Coordinator, Clinical Supervisor or Mental Health Director based upon requests of the Facility administrative staffs, rules and regulations governing the employee’s license.

Provision of training in human behavior and/or mental health issues to institutional staff in accordance with the training program and as assigned by the Mental Health Coordinator, Clinical Supervisor, Mental Health Director.

Completion of relevant clinical documentation in the health record regarding the consumer’s participation in mental health treatment.

Completion of regular reports of the employee’s activities in accordance with the policies and procedures.



Must be licensed as an LGSW or LGPC

Master's Level degree required

Training or experience in a health care, human service field or equivalent experience preferred.

Physical Requirements:

Able to lift at least fifty pounds (50).
