The CABHI team is responsible for conducting outreach to individuals throughout the city who are experiencing chronic homelessness. MBI’s team consists of Outreach & Engagement Specialists, Peer Specialists, SOAR Specialist and Supported Employment Specialist. Once the CABHI team member assesses the individual on the street or in shelter he or she is entered into the city-wide Homeless Management Information System (HMIS). Using HMIS, the coordinated entry team of The Community Partnership facilitates meetings where match recommendations are made to housing opportunities. The CABHI team then assists the individual with obtaining any vital documents he or she may need (ID, birth certificate, applying for SSI benefits, etc.) so that he or she will be able to lease their own housing unit. In addition, the CABHI team helps the individual enroll in a Core Service Agency or Substance Abuse treatment if he or she chooses to do so. If the individual would like to seek employment they will be able to receive additional support from the CABHI Supported Employment Specialist.
